Decedent claim documents

The claimant must be the executor, administrator, or next of kin: a copy of the death certificate, proof of the owner's Social Security number, and an executor/administrator's court appointment (filed with the Treasury within 60 days of issuance).

Small estate option

Heirs of an estate with personal property totaling $30,000 or less (KRS 395.455) may use a court petition to dispense with full administration (form AOC-830). Per 20 KAR 1:040, the Treasury accepts a small estate affidavit as an alternative to a full court appointment when documenting authority to claim.
