After a death

Documentation for final payment

Mail a letter identifying the deceased claimant, the requester's relationship to them, and a return address, together with a copy of the death certificate and proof of executorship (Letters Testamentary or Letters of Administration), to: Kansas Department of Labor, 401 SW Topeka Blvd., Topeka, KS 66603-3182. KDOL legal staff will evaluate the claim on receipt.
