State Unemployment Benefits in Nevada

The Nevada Department of Employment, Training and Rehabilitation administers unemployment insurance. Benefits due a deceased claimant may be paid to the person or persons the Administrator finds legally entitled to them, on an affidavit executed by that person (NRS 612.360).

Benefits due a deceased claimant
Benefits due a deceased claimant may be paid to whoever appears to the Administrator to be legally entitled to them, in accordance with authorized regulations, and are paid on an affidavit executed by the person claiming entitlement. The affidavit discharges the Administrator from further liability (NRS 612.360).
Form NUCS-4172
DETR publishes Form NUCS-4172 ("Affidavit Concerning Deceased or Incompetent Person") for an heir to request a duplicate 1099-G tax document when no executor or administrator has been appointed. The affiant signs before a notary and returns the form, with supporting documents, to the Employment Security Division, Attn: Monetary Unit, 500 E. 3rd St., Carson City, NV 89713-0045.
Who may request the 1099-G
An heir of the deceased claimant who attests that no executor or administrator has been appointed or is contemplated and that there is no other person who would be an heir. The affiant states their relationship to the deceased on Form NUCS-4172.
