The affiant signs before a notary and submits supporting documents to the Employment Security Division Monetary Unit, 500 E. 3rd St., Carson City, NV 89713.

An heir of the deceased claimant who can attest that no executor or administrator has been appointed or is contemplated, and that they are the sole heir. The affiant states their relationship to the deceased on Form NUCS-4172.
