Call the UI Contact Center at 1-877-345-3484. There is no online form for reporting a claimant death — the process is phone-only.
Documentation for final payment
Under ORS 293.495, the person claiming the final payment submits an affidavit to the Department confirming: the claimant has died, the estate will not be probated, the total state money owed does not exceed $10,000, and the claimant's relationship to the deceased. No additional agency-specific form is prescribed by statute.
Who may claim
Under ORS 293.490, payment goes first to a surviving spouse, then to a trustee of the decedent's revocable trust, then to children and descendants, then to parents, then to siblings, then to nieces and nephews. An executor or personal representative of the estate may also act on behalf of the deceased claimant per OED administrative practice (cross-referencing ORS 293.490). The maximum final payment is $10,000.
