After a death
Report the death so payments stop. An authorized representative can request a deceased claimant's 1099-G by mailing the death certificate, the representative's relationship, and proof of authority to DWD.

Documentation for final payment
DWD's published guidance covers the 1099-G request: mail a statement that the request is for a deceased individual, your name, address, phone, and relationship to the deceased, the deceased's name and last four digits of their SSN, a copy of the death certificate (all but the last four SSN digits removed), and a signed letter showing you are authorized. DWD has not published separate documentation requirements for claiming a final benefit payment; contact DWD directly to confirm what is needed.
