Steps to take

  1. Report the claimant's death to the Indiana Department of Workforce Development so benefit payments stop.
  2. Ask DWD whether a final payment is owed for eligible weeks before death and what documentation is required.
  3. Request the deceased claimant's 1099-G for tax filing by mailing DWD a request that includes:
    • A statement that the request is for a deceased individual
    • Your name, address, phone, and relationship to the deceased
    • The deceased's name and last four digits of the Social Security number
    • A copy of the death certificate and a signed letter showing you are authorized
