- After a death

- Required documents
- A relative with first-hand knowledge submits a certification, an affidavit of the relationship to the deceased, and a certified copy of the death certificate, within nine months of the date of death (56 Ill. Adm. Code 2830.200).
- Reporting a death
- Report by submitting the deceased-claimant certification form to a local IDES unemployment office in person or by certified mail. Forms are available at any local IDES office or by calling Claimant Services at 1-800-244-5631. There is no online death-reporting portal (56 Ill. Adm. Code 2830.200).
- Documentation for final payment
- A completed IDES certification form, an affidavit of relationship to the deceased signed by the submitter, and a certified copy of the death certificate — all submitted within nine months of the date of death. No Letters Testamentary are required; the check is issued to "The Estate or Heirs of [Claimant]" rather than to a named individual.
- Who may claim
- Any relative with first-hand knowledge of the claimant's death may submit the certification. Payment is made payable to "The Estate or Heirs of" the deceased claimant; a check is not made payable to more than one payee nor to a named individual (56 Ill. Adm. Code 2830.200).
