Steps to take when someone dies
- •Online: Order at vitalrecords.utah.gov (next business day if ordered before 1:00 PM with expedite fee)
- •By mail: Mail completed application with payment and ID. Allow 6 weeks processing
- •In person: Visit Local Health Department (no walk-in window at state office)
- •One current primary photo ID (e.g. driver license, state ID, passport), or two secondary IDs
- •Mail orders: a photocopy of the front and back of the ID
- •Proof of relationship for family members (e.g. birth or marriage certificates)
- •Legal representatives: a letter of legal need from the entity requiring the certificate
