•By mail: Mail the State application to the State office (money order to "Treasurer, State of CT") or the Town application to the town of death (money order to the town)
•In person: Visit the town vital records office where the death occurred or where the decedent lived, or the State Vital Records Office walk-in window (photo ID needed to enter the building)

Include what the office requires with the order:

•Proof of identity and relationship for copies with Social Security number
•Valid photo ID and proof of relationship for the veteran fee waiver
