No statewide form. A.R.S. § 14-3971(B)(1)-(4) sets what the affidavit must state, and the affidavit is drafted to those requirements; the required elements print with the document as a checklist.
$200,000, per A.R.S. §§ 14-3971, 14-3972. This tool checks the entered estate value against the limit and does not prepare an affidavit for an estate over it.
30 days after the death (A.R.S. §§ 14-3971, 14-3972). The affidavit states that the waiting period has elapsed, so it cannot be signed earlier.
A person claiming to be the successor of the decedent; the affidavit is made by or on behalf of the successor. (Separately, § 14-3971(A) lets the surviving spouse collect up to $5,000 of unpaid wages by affidavit at any time after death.) A.R.S. § 14-3971(A), (B).
The person indebted to the decedent or having possession of the decedent's tangible personal property or an instrument evidencing a debt, obligation, stock, or chose in action (bank, employer, transfer agent for securities re-registration, or the motor vehicle division for title transfer).
The person paying, delivering, transferring, or issuing property pursuant to the affidavit is discharged and released to the same extent as if dealing with a personal representative, and need not inquire into the truth of any statement in the affidavit; the recipient is answerable and accountable to any personal representative of the estate or any other person having a superior right (§ 14-3972(A)).
Real property has a separate COURT-FILED affidavit of succession to real property (§ 14-3971(E)): filed no sooner than six months after death in the superior court of the county of domicile (or where the property sits), subject to the separate real-property value limit; the registrar issues a certified copy that is recorded with the county recorder (§ 14-3971(F)).
