  2. Register for a free account and file a claim as the heir or estate representative, submitting the printed claim form with supporting documentation:
    • Proof of death
    • Proof of authority to act for the estate, such as Letters Testamentary or Letters of Administration
    • A validly executed Unclaimed Property Power of Attorney (Form UP-1061) if a third party files on behalf of the owner
  3. Track the claim status online; the Department issues payment by check, generally within 30 to 90 days.
