Steps to take

  1. Search the Arizona Department of Revenue unclaimed property listings (via azdor.gov/unclaimed-property and missingmoney.com) for the deceased's name and prior addresses.
  2. File the claim as the estate representative or heir with proof of death and proof of authority:
    • A copy of the owner's death certificate
    • For an estate of $75,000 or less, an Affidavit for Collection of Personal Property
    • For a probated estate, the personal representative's Letters of Office certified within the last 60 days
    • A copy of the will and any trust referenced in it, if one exists
  3. Submit the completed claim form and supporting documents to the Unclaimed Property Unit.
  4. Allow up to 90 days for the Unit to verify and process the claim; it may request additional documentation before paying.
