Steps to take

  2. Contact the DETR UI call center to ask whether a final payment is owed for eligible weeks before death and what documentation is required.
  3. Return any payment issued for weeks after the date of death.
  4. If a duplicate 1099-G is needed and no executor or administrator has been appointed, complete Form NUCS-4172 before a notary, attach supporting documents, and mail to the Employment Security Division Monetary Unit at 500 E. 3rd St., Carson City, NV 89713.
