Global Atlantic lists these among its required documents: Completed Life Insurance Claim Form / Statement of Beneficiary (Form 216779 for Forethought Life; Form 17003 for Accordia Life; Form 17003(FANY) for First Allmerica); Certified death certificate (required for underwritten policies, policies within 2 years of issue, accidental death, or suicide); Government-issued photo ID for the beneficiary; Policy or contract number and deceased's personal information. 30-60 days from receipt of all required documentation. Preneed claims may be processed more quickly when coordinated through the funeral home. State laws may require payment within a specific timeframe once all documentation is received.
Global Atlantic provides its own letter-of-instruction form. We complete that official form with your information; you print, sign, and send it.
It depends on the capacity you are acting in. An executor or administrator encloses Letters Testamentary (when there is a will) or Letters of Administration (when there is not); a successor trustee encloses a certificate of trust; a successor under a small estate encloses that state’s small estate affidavit.
