  • For life insurance policies: download the Beneficiary Change Request form (Form 13977) from globalatlantic.com/life-insurance/resources-and-forms
  • For survivorship life insurance products: use Form 18185 (Change of Beneficiary for Indexed Survivorship UL Rider)
  • If additional beneficiary space is needed: use Form 17963 (Additional Beneficiaries Designation Supplement)
  • For annuity contracts: download the Change of Beneficiary form (Form FA5014) from the annuity forms library at professionals.globalatlantic.com/resources/forms

Complete the form with beneficiary details:
  • Provide primary and contingent beneficiary information including full legal names, dates of birth, relationships, Social Security numbers, and percentage allocations
  • For trusts: provide the full trust name, date established, trustee name(s), and EIN; submit a copy of the trust document for the company's file

  • Trust name, date established, and tax ID (EIN or SSN)

Special Requirements

  • Trusts can be named as beneficiaries on life insurance and annuity products
  • A copy of the trust document must be submitted for the company's file when naming a trust as beneficiary
  • If a corporate entity is the beneficiary, a recent Corporate Resolution must be submitted unless already on file
  • Primary and contingent beneficiary percentage allocations must each total 100%
  • Additional beneficiary forms are available if more space is needed (Form 17963 Additional Beneficiaries Designation Supplement; Form 18185 for survivorship UL rider products)
  • Beneficiary designations override instructions in a will
  • Spousal consent may be required in community property states
