Cigna lists these among its required documents: Certified death certificate (original or certified copy); Completed claim form (provided by New York Life GBS or Cigna); Policy or certificate number and employer/group name; Deceased employee's full legal name, date of birth, Social Security number, and date of death. Claims are typically processed within 5-10 business days after receipt of all required documentation. Beneficiaries receiving payments of $5,000 or more from New York Life GBS gain access to a Survivor Assurance interest-bearing account plus counseling and financial support services.

It depends on the capacity you are acting in. An executor or administrator encloses Letters Testamentary (when there is a will) or Letters of Administration (when there is not); a successor trustee encloses a certificate of trust; a successor under a small estate encloses that state’s small estate affidavit.
