Brand change

Formerly Allstate Life Insurance Company. Allstate sold its life insurance business to Blackstone-managed entities in November 2021 (~$4 billion), and the company was renamed Everlake Life Insurance Company. In-force policies originally issued by Allstate Life are now serviced under the Everlake brand; servicing was administered by Allstate through a transition and migrated to Transaction Applications Group, Inc. (TAG), an NTT Data company, in phases completing June 8, 2026. This is the current active record. Effective November 2021.

The procedures below reflect Everlake Life's accounts. Account servicing may transfer as the change takes effect.

Filing a death claim on a Everlake Life policy begins with notifying the Life and Annuity Claims (via TAG / NTT Data) at 1-844-953-0347. Life insurance claims are separate from probate—proceeds transfer directly to named beneficiaries regardless of whether the estate goes through court. The timeline and documentation requirements depend on the policy type and cause of death.

Everlake Life offers an online claims portal that makes the initial filing process more straightforward. Survivors can also initiate claims by phone or by mailing documentation directly.

Death claim process

The death claim process at Everlake Life works as follows:

Filing a claim

1
Identify which Everlake claims team handles the product -- Everlake routes to FOUR separate teams, and sending a claim to the wrong one delays it:
  • Life insurance, GoodForLife, and annuities: the Lincoln, NE service center (1-844-953-0347 life / 1-844-953-2815 general annuity / 1-833-879-0774 VUL and Helmsman/Horizon/RightFit)
  • Structured settlement annuities: Everlake Structured Settlement Claims, 1-833-879-0775
  • Pension buyout (annuity buyout) contracts: Everlake Pension Buyout Claims, 1-833-879-0775
  • Long-term care: Long Term Care Administrative Office, Woodland Hills, CA, 1-888-505-8744
2
Notify Everlake of the death through one of these channels:
  • File online through the Everlake Customer Portal at https://customer.everlakelife.com/guest/claims/file-a-claim
  • Call 1-844-953-0347 (life insurance) or 1-844-953-2815 (annuities)
  • Call 1-833-879-0774 (variable universal life or Helmsman/Horizon/RightFit annuities)
  • Call 1-844-953-0347 (GoodForLife life policies, migrated from Allstate to TAG effective June 8, 2026) or follow your TAG transition letter
3
Provide the deceased's policy or contract number, date of death, and beneficiary information
4
Receive and complete the claim form packet mailed by Everlake / TAG
5
Submit the completed claim form with required documents:
  • Mail to Everlake Life Claims, P.O. Box 83328, Lincoln, NE 68501 (life/general annuity)
  • Mail to Everlake Customer Support, P.O. Box 82207, Lincoln, NE 68501 (VUL/variable annuity)
  • Overnight: Everlake, 777 Research Dr., Lincoln, NE 68521
  • Fax (life/general annuity): 1-833-636-0034
  • Fax (VUL/variable annuity): 1-833-636-0035
6
Everlake / TAG evaluates the claim and contacts beneficiaries if additional information is needed
7
Decide how the approved death benefit is paid -- Everlake may settle it into an Everlake Advantage Account rather than mailing a lump-sum check:
  • The Everlake Advantage Account is an interest-bearing retained-asset settlement account opened in the beneficiary's name and serviced by The Northern Trust Company, not by TAG (1-855-539-6487, https://www20310.ntrs.com/raw/public/als/login)
  • The beneficiary can withdraw the entire balance at any time
  • A balance left in the account is the BENEFICIARY's own asset: if that beneficiary later dies with money still in it, the balance passes through the beneficiary's own estate, so sweep it into an account that carries its own beneficiary designation or into a trust

Required Documents

  • Completed claim form (provided in the claim packet)
  • Certified copy of the death certificate
  • Government-issued photo identification for each beneficiary
  • Trust documents or Certificate of Trust (if trust is named as beneficiary)
  • Letters Testamentary or Letters of Administration (if estate is beneficiary)
  • W-9 for the beneficiary (for tax reporting on annuity proceeds)
  • Additional documentation for minor, foreign, or qualified plan beneficiaries

What to know at this institution

Claims for Everlake products are processed by Transaction Applications Group, Inc. (TAG), an NTT Data company. Under the March 16, 2026 transition FAQ, GoodForLife was still serviced by Allstate; Everlake's current service and claims pages now state that "effective June 8, servicing for all remaining Everlake products has migrated from Allstate to TAG," and GoodForLife is no longer listed as Allstate-serviced -- GoodForLife holders should follow their TAG transition letter or use the life claims line 1-844-953-0347. Life claims fax: 1-833-636-0034. Annuity claims fax (Helmsman/Horizon/RightFit): 1-833-636-0035. Overnight delivery for life and annuity claims: Everlake, 777 Research Dr., Lincoln, NE 68521. IMPORTANT ROUTING: Everlake's block is broader than life and annuity, and it does NOT run every claim through the Lincoln, NE service center. Structured settlement claims go to Everlake Structured Settlement Claims, P.O. Box 81007, Lincoln, NE 68501, phone 1-833-879-0775, fax 1-833-636-0034. Pension buyout (annuity buyout) claims go to Everlake Pension Buyout Claims, P.O. Box 81007, Lincoln, NE 68501, phone 1-833-879-0775, fax 1-833-636-0034. Long-term care is administered from an entirely different office -- Everlake Life Insurance Company, Long Term Care Administrative Office, PO Box 4243, Woodland Hills, CA 91365-4243, phone 1-888-505-8744, fax 1-818-887-4595 -- and because long-term care is a living-benefit reimbursement policy rather than a death benefit, the death is reported there only to close the policy and stop premium billing, not to collect a payout. Finally, an approved life insurance death benefit is commonly settled into an Everlake Advantage Account, a retained-asset settlement account opened in the beneficiary's name and serviced by The Northern Trust Company (P.O. Box 92987, Chicago, IL 60675-2987; overnight Lockbox #92987, 5503 N. Cumberland Ave, Chicago, IL 60656; 1-855-539-6487; https://www20310.ntrs.com/raw/public/als/login) rather than paid out as a mailed check.

Expected timelines at Everlake Life: Processing times vary by state requirements. Contact Everlake for estimated timelines specific to your claim. Delays are almost always caused by incomplete paperwork—gathering all required documents before filing the initial claim helps avoid back-and-forth.

Everlake Life requires several documents to process a claim, including Completed claim form (provided in the claim packet), Certified copy of the death certificate, and Government-issued photo identification for each beneficiary, and additional documentation depending on the account type. Certified copies are typically needed—photocopies are generally not accepted for death certificates or court documents.

Frequently asked questions

Policy servicing and claims for Everlake products are handled by Transaction Applications Group, Inc. (TAG), an NTT Data company. The migration ran in phases: under the March 16, 2026 transition FAQ most products had moved to TAG while GoodForLife remained with Allstate, and Everlake's current service pages state that "effective June 8, servicing for all remaining Everlake products has migrated from Allstate to TAG." Policyholders and contract holders received notification letters with updated contact information and instructions. Access the TAG-administered Customer Portal at https://customer.everlakelife.com/ for online self-service, beneficiary updates, and claim filing, or call the life line 1-844-953-0347.

Yes. Everlake life insurance policy ownership can be transferred to an ILIT. Contact Everlake at 1-844-953-0347 (life insurance) or 1-833-879-0774 (variable universal life) to request the required ownership transfer forms. The three-year lookback rule under IRC Section 2035 applies: if the insured dies within three years of the ownership transfer, the death benefit may be included in the taxable estate for federal estate tax purposes.

Beneficiaries can file a claim online through the Everlake Customer Portal at https://customer.everlakelife.com/guest/claims/file-a-claim or by calling 1-844-953-0347 (life), 1-844-953-2815 (general annuities), or 1-833-879-0774 (VUL or Helmsman/Horizon/RightFit annuities). You will need the deceased's policy or contract number, date of death, and a certified death certificate. Everlake / TAG mails a claim packet, which the beneficiary completes and returns to Everlake Life Claims, P.O. Box 83328, Lincoln, NE 68501 (or fax 1-833-636-0034 for life/general annuity).

The Everlake Advantage Account is a retained-asset settlement account: rather than mailing a lump-sum check, Everlake can settle an approved life insurance death benefit into an interest-bearing account opened in the beneficiary's name. It is serviced by The Northern Trust Company, not by TAG -- reach it at 1-855-539-6487, online at https://www20310.ntrs.com/raw/public/als/login, or by mail at Everlake Advantage Account, c/o The Northern Trust Company, P.O. Box 92987, Chicago, IL 60675-2987. You can withdraw the entire balance at any time. The estate planning consequence is easy to miss: money sitting in an Advantage Account is your own asset, and the account is not a policy with a beneficiary designation of its own. If you die with a balance still in it, that balance passes through your estate and can be subject to probate. If you intend to hold the money rather than spend it, move it into an account that carries its own beneficiary or POD designation, or into your revocable trust.

No. Everlake routes these to entirely separate teams, and sending the claim to the main life and annuity service center will delay it. Structured settlement claims go to Everlake Structured Settlement Claims, P.O. Box 81007, Lincoln, NE 68501, phone 1-833-879-0775, fax 1-833-636-0034. Pension buyout (annuity buyout) claims go to Everlake Pension Buyout Claims at the same P.O. Box 81007 address and the same 1-833-879-0775 line. Long-term care is administered from a different state entirely: Everlake Life Insurance Company, Long Term Care Administrative Office, PO Box 4243, Woodland Hills, CA 91365-4243, phone 1-888-505-8744, fax 1-818-887-4595. Note that long-term care is a reimbursement policy paid during the insured's life, not a death benefit, so a death is reported there to close the policy and stop premium billing rather than to collect a payout. For structured settlements and pension buyouts, what survives the death is fixed by the settlement agreement or the payment form the retiree elected -- guaranteed payments continue to the named beneficiary, while life-contingent payments end at death.

Everlake Life's Life and Annuity Claims (via TAG / NTT Data) can be reached by phone at 1-844-953-0347, email at ELCustomerService@everlake.nttdata.com, and fax at 1-833-636-0034 for questions throughout the claims process.

If the deceased held multiple Everlake Life policies, each may require a separate claim or have different documentation requirements. The Life and Annuity Claims (via TAG / NTT Data) can confirm which accounts require individual attention and which can be processed together.
