Legal & General America lists these among its required documents: Completed Proof of Death Claimant's Statement (each beneficiary must complete one; original signatures required); Certified death certificate containing cause and manner of death (cannot show cause as "pending"; will not be returned); Original policy document (if available; indicate on Statement form if unavailable); Government-issued ID for the beneficiary. Claims of $50,000 or less may qualify for accelerated processing. Larger claims typically take 30-60 days from receipt of complete documentation. Banner Life reports a 99% claims payment rate.

It depends on the capacity you are acting in. An executor or administrator encloses Letters Testamentary (when there is a will) or Letters of Administration (when there is not); a successor trustee encloses a certificate of trust; a successor under a small estate encloses that state’s small estate affidavit.
