American Express lists these among its required documents: Deceased Letter of Instruction (LOI) form; Death certificate (copy only; originals cannot be returned); For individual accounts with no POD beneficiary: Letters Testamentary, Letters of Administration, or notarized state-specific Small Estate Affidavit; For individual accounts with POD beneficiaries: an Affidavit of Identity for each beneficiary. It depends on the capacity you are acting in. An executor or administrator encloses Letters Testamentary (when there is a will) or Letters of Administration (when there is not); a successor trustee encloses a certificate of trust; a successor under a small estate encloses that state’s small estate affidavit.
