A death claim on a United American policy is filed through the Life Claims (1-800-331-2512). Because insurance proceeds pass directly to named beneficiaries, this process is separate from probate. The required documentation and timeline vary by policy type.

United American provides an online portal for initiating death claims, which can simplify the initial notification and document submission process. Claims can also be started by phone or by mailing the required documents.

Death claim process

To file a claim after an account holder's death, here is what United American requires:

Identify what the deceased actually held before filing anything
  • United American is primarily a Medicare Supplement and supplemental health carrier; most of its in-force policies pay no death benefit at all
  • Medicare Supplement (Plans A, B, C, D, F, G, K, L, M, N), cancer, critical illness, accident, and group retiree health policies pay no death benefit and terminate at the insured death - there is nothing for a beneficiary to claim
  • Only the whole life policies (final expense whole life, juvenile whole life) generate a death benefit claim
  • The policy number and the plan name on the premium notice or ID card tell you which one you are dealing with

If the deceased held Medicare Supplement or supplemental health coverage, cancel it and stop the premium
  • Call Customer Care at 1-800-331-2512 (Monday-Friday, 7:30 a.m. - 5:00 p.m. CT) to report the death and cancel the policy
  • Cancelling stops the recurring premium draft or bank draft; premiums are not automatically stopped by the death certificate reaching Medicare
  • Ask Customer Care about any premium paid for coverage past the date of death - United American does not publish a refund schedule, so the refund of unearned premium is handled case by case through Customer Care
  • Any refund check is an estate asset payable to the estate, not to a beneficiary - it goes to whoever is appointed to collect the estate assets
  • Providers may still be submitting bills for care given before the death; United American receives most Medicare Supplement claims electronically from Medicare, so the survivor usually does not have to file them, but keep the policy account open until the last pre-death bill has cleared

Sending a life claim to the health claims box is the most common way these claims stall. Beneficiary designations on life insurance override instructions in a will and pass outside probate. A Medicare Supplement or supplemental health policy is not an estate asset and has no death benefit - the only estate consequences are cancelling the coverage and collecting any refund of premium paid past the date of death, which is payable to the estate.

Only the whole life policies. Final expense whole life and juvenile whole life have a death benefit, so they can name a trust as beneficiary and can be assigned to an Irrevocable Life Insurance Trust (ILIT) as owner. Everything else United American sells - Medicare Supplement Plans A through N, cancer, critical illness, accident, and group retiree health coverage - pays no death benefit, so a trust cannot own it, be named on it, or receive anything from it. If a United American policy shows up in an estate inventory, check the plan name before assuming it is life insurance; most of the company in-force book is Medicare Supplement.

Claims are paid and administered out of the McKinney, Texas home office under the United American name, so a death certificate and claim form addressed to United American at P.O. Box 8076 is correct even though the online portal carries Globe Life branding. Do not send the claim to another Globe Life subsidiary - a policy issued by another Globe Life company such as Globe Life And Accident Insurance Company or Family Heritage Life is a separate insurer with a separate claim address.
