  • OneAmerica will mail an acknowledgment letter and a Life Insurance Claims Packet or Deferred Annuity Claims Packet to the beneficiary

Complete and submit the claims packet:
  • Complete and sign the required claim forms included in the packet, including the Beneficiary Claim Form (Form R-33492)
  • Submit the completed claim packet along with a certified death certificate and the original policy/contract if available

  • OneAmerica will advise the beneficiary in writing and by phone if additional claim requirements are needed
  • Once all required documents are received, OneAmerica processes the claim within 5 business days

Asset Care combines whole life insurance with long-term care benefits. The guaranteed death benefit can be accessed tax-free to help pay for qualifying long-term care services under the Pension Protection Act. If you do not require long-term care support, the cash value remains accessible for emergencies or passes to your beneficiaries as a death benefit. A trust can be named as beneficiary to receive the death benefit. Premiums cannot be increased once the policy is issued. OneAmerica completed the sale of its full-service retirement plan recordkeeping business to Voya Financial on January 1, 2025. Approximately $60 billion in plan assets across roughly 8,000 retirement plans transitioned to Voya. If the deceased held a 401(k), 403(b), or other workplace retirement plan that was previously serviced by OneAmerica, the death claim is now handled by Voya.
