What to do when a Horace Mann account holder dies

Contact Horace Mann — 7-step process, 9 required documents, and horace mann publishes no fixed payout timeline. a life insurance death benefit is typically paid as an income-tax-free lump sum once the completed packet and certified death certificate are in. delays come from three places: an incomplete packet, a policy still inside its two-year contestability period (which triggers a medical-records review), and a death whose circumstances need investigation. a 403(b) or 457(b) annuity claim is on a different clock entirely — the beneficiary's deadline is the secure act's, and a trust beneficiary's see-through documentation is due to horace mann by october 31 of the year after the participant's death.

A death claim on a Horace Mann policy is filed through the Horace Mann Claims — life insurance and annuity death claims (one intake for both) (1-800-999-1030). Because insurance proceeds pass directly to named beneficiaries, this process is separate from probate. The required documentation and timeline vary by policy type.

Death claims at Horace Mann can be started through an online portal, which streamlines the initial notification and document upload. Phone and mail options are also available.

Death claim process

The death claim process at Horace Mann works as follows:

Filing a claim

1
Take inventory first — an educator with Horace Mann usually has more than one thing in force, and they pay out through different doors:
  • Life insurance (Life by Design Term, Cash Value Term, Life Select Whole Life, Pathfinder IUL) — pays the named beneficiary as an income-tax-free death benefit
  • A 403(b), 457(b), or 401(a) retirement ANNUITY (PRP annuities, MY Guaranteed Solution II, Retirement Advantage) — pays the named beneficiary, but as taxable retirement money on a SECURE Act clock, not as a tax-free death benefit
  • Supplemental cancer / heart / hospital / accident coverage — usually pays the policyholder in life, though an accident policy may carry an accidental death benefit
  • Auto and home policies — no death benefit, but they still have to be continued or cancelled by the estate
  • A separate state TRS / STRS / PERS pension — not a Horace Mann product at all, and it has its own survivor claim
2
Order several certified death certificates before you start; every one of the above wants its own.
3
Report the death once. Horace Mann uses a single intake for life AND annuity claims:
  • Online at https://www.horacemann.com/claims/death-claims (the fastest route)
  • Or by phone at 800-999-1030, where Horace Mann publishes live claims help 24/7
  • Disability / waiver-of-premium claims do NOT go here — those are 800-356-9601
4
Horace Mann forwards the report to a regional office; a representative there contacts the beneficiary and mails the claim packet to the NAMED BENEFICIARIES.
5
Complete the packet and return it with a certified death certificate and beneficiary ID. Expect a medical-records request if the policy is inside its two-year contestability period.
6
Choose the payout — and understand that the choice is not the same on the two products:
  • LIFE INSURANCE: typically a lump sum, income-tax-free to the beneficiary
  • A 403(b) / 457(b) ANNUITY: the beneficiary's options are governed by the SECURE Act, not by Horace Mann. A non-spouse beneficiary who is not an Eligible Designated Beneficiary must empty the account within 10 years; a surviving spouse has more options, including a rollover. Taking the whole annuity as a lump sum is usually the worst of them, because it is taxable income in a single year
  • Ask Horace Mann to confirm whether the participant had reached their Required Beginning Date, because that determines whether annual RMDs are also required during the 10-year window
7
Close out the property side: contact Horace Mann about the deceased's auto and home policies to keep the estate's property insured while it is being settled, or to cancel for a pro-rated refund.

Required Documents

  • Policy or contract number (life policy, annuity / supplemental contract, group certificate) and the deceased's full information
  • Certified death certificate — order several
  • Completed forms from the Horace Mann claim packet (mailed to the named beneficiaries; no public download)
  • Government-issued ID for the beneficiary
  • Certification of Trust and trustee ID, if a trust is the beneficiary — and for a 403(b)/457(b) annuity, furnished to Horace Mann by October 31 of the year after the participant's death to preserve see-through treatment
  • Proof of guardianship or custodianship, if a beneficiary is a minor
  • Medical records or a physician's statement, if the policy is within its two-year contestability period
  • Police or accident report, if the death was accidental (also relevant to an accidental death benefit rider)
  • Funeral home name, address, and phone (asked for on the Death Claims intake form)

What to know at this institution

Horace Mann reports life and annuity deaths through one intake (the Death Claims form at horacemann.com/claims/death-claims, or 800-999-1030 with live claims help 24/7), then routes the claim to a regional office whose representative contacts the beneficiary and mails the claim packet to the NAMED BENEFICIARIES — so the person who reports the death is often not the person who receives the paperwork. There is no downloadable death-claim form. The intake form deliberately asks whether the deceased also held Horace Mann auto or property policies, which is the hook for cleaning up the property side. Three lines are NOT the death-claim line: disability and waiver-of-premium claims (800-356-9601), Retirement Advantage account servicing (800-677-2363, Monday-Friday 8 a.m.-5 p.m. ET), and supplemental policies issued by National Teachers Associates (888-671-6771). The substantive difference for an educator: a life death benefit is a tax-free lump sum, while a 403(b)/457(b) annuity is inherited retirement money governed by the SECURE Act — a non-spouse, non-EDB beneficiary must empty it within 10 years, and a trust beneficiary must get its documentation to Horace Mann by October 31 of the year after the death. And nothing filed with Horace Mann touches the state TRS / STRS / PERS pension, which the survivor must claim separately.

Processing timelines at Horace Mann: Horace Mann publishes no fixed payout timeline. Incomplete documentation is the most common cause of delays—submitting all required documents with the initial claim helps avoid additional processing time.

Horace Mann requires several documents to process a claim, including Policy or contract number (life policy, annuity / supplemental contract, group certificate) and the deceased's full information, Certified death certificate — order several, and Completed forms from the Horace Mann claim packet (mailed to the named beneficiaries; no public download), and additional documentation depending on the account type. Certified copies are typically needed—photocopies are generally not accepted for death certificates or court documents.

Frequently asked questions

Yes, in most cases. Under the SECURE Act (effective for deaths after December 31, 2019) and SECURE 2.0, a non-spouse designated beneficiary who is not an "Eligible Designated Beneficiary" (EDB) must withdraw the entire inherited 403(b) annuity by December 31 of the tenth year after the participant's death. Final IRS regulations issued in July 2024 also require annual Required Minimum Distributions during years 1-9 of that 10-year window if the participant died on or after their Required Beginning Date. EDBs (surviving spouse, minor child of the participant until majority, disabled or chronically ill beneficiary, beneficiary less than 10 years younger than the participant) can still use the life-expectancy stretch. Eligible rollovers to an inherited IRA preserve the same 10-year clock.

Only if the trust qualifies as a "see-through" (look-through) trust under Treas. Reg. 1.401(a)(9)-4. Four requirements: (1) the trust is valid under state law, (2) the trust is irrevocable or becomes irrevocable on the owner's death, (3) all trust beneficiaries are identifiable individuals, and (4) trust documentation (or a Certification of Trust listing trust beneficiaries) is provided to Horace Mann by October 31 of the year after the participant's death. If the trust qualifies, the underlying beneficiaries are treated as designated beneficiaries — non-EDB individuals still get the 10-year rule, EDBs get life expectancy. If the trust fails see-through status, the entire balance pays out under the 5-year rule (death before Required Beginning Date) or the participant's remaining single life expectancy.

A 403(b) annuity is a supplemental defined-contribution account that sits alongside, not inside, your state pension. The state TRS / STRS / PERS defined-benefit pension is governed by its own statutes and beneficiary forms; nothing you do on a Horace Mann 403(b) beneficiary form affects who receives the state pension survivor benefit. Each system has independent forms and quirks — for example, Texas TRS requires its own TRS 15 Designation of Beneficiary, California STRS uses the SR0387 form, and most state systems default the survivor benefit to a member's spouse unless the spouse waives. If you are divorced, remarried, or living apart from a spouse, update both your Horace Mann beneficiary and the state system beneficiary; one does not override the other, and a Qualified Domestic Relations Order (QDRO) governing the state pension does not automatically reach your private 403(b) annuity.

No. Auto and home property and casualty policies through Horace Mann are insurance on assets, not death benefits. They carry no beneficiary designation and are not retitled to a trust as part of estate planning. Horace Mann's own Death Claims intake form asks whether the deceased also held auto or property policies, which is the prompt to deal with them: the executor or successor trustee contacts Horace Mann to be added as an authorized contact, updates billing, and either keeps coverage in force on the vehicle or home while the estate holds it, or cancels for a pro-rated refund. Do not let the property coverage lapse on a house that is sitting empty during probate. Coverage on a home being deeded into a revocable trust generally needs the trust added to the policy — call the Horace Mann agent before the deed is recorded, not after.

Yes. Permanent life insurance (Life Select Whole Life, Cash Value Term, Pathfinder Indexed Universal Life) and term policies can be assigned to an ILIT to remove the death benefit from your taxable estate. Request an assignment of ownership form from Horace Mann at 855-393-6467 and submit it together with a copy of the executed trust agreement (or a Certification of Trust) listing the trustee as the new policy owner. Under IRC Section 2035 the three-year lookback rule pulls the death benefit back into your taxable estate if you die within three years of transferring an existing policy; this is why advisors often recommend having the ILIT purchase the policy at issuance instead. Once transferred, the grantor permanently gives up the right to change beneficiaries, take loans, or surrender the policy. Horace Mann does not draft ILITs; this is attorney work.

Horace Mann's Horace Mann Claims — life insurance and annuity death claims (one intake for both) can be reached by phone at 1-800-999-1030 for questions throughout the claims process.

Multiple Horace Mann policies may mean multiple claims. Some account types can be processed together, but others require their own documentation. Check with the Horace Mann Claims — life insurance and annuity death claims (one intake for both) to confirm what applies.
