When an insured person dies, the beneficiary or executor should contact F&G's F&G Service Center (Claims) at 1-888-513-8797 to start the claims process. Insurance proceeds are paid directly to the named beneficiary and do not go through probate. How quickly the claim is processed depends on the policy type, documentation, and cause of death.

Death claims at F&G can be started through an online portal, which streamlines the initial notification and document upload. Phone and mail options are also available.

Death claim process

The death claim process at F&G works as follows:

Report the death to F&G:
  • Contact the F&G Service Center at 888-513-8797
  • Provide the deceased's personal information, date of death, and policy or contract number

F&G will send a claims packet to the beneficiary with all required forms

Complete and gather required documentation:
  • Each beneficiary completes their Claimant Statement (included in claims packet)
  • Obtain a certified death certificate issued by the state in which the deceased passed away
  • If trust is beneficiary: trustee(s) complete the Claimant Statement and provide the trust Tax Identification Number

Living benefits claims (chronic, terminal, or critical illness) can be filed by calling 866-702-2194, option 5. Overnight mailing address: F&G Service Center, 777 Research Drive, Lincoln, NE 68521. Faxed documents take 3 business days to enter the processing system. Fixed and fixed indexed annuities that have not been annuitized have surrender charges waived upon the annuitant's death, and the remaining account value passes to named beneficiaries.

Processing timelines at F&G: Claims are typically processed within 30-45 days of receipt of all required documentation. Allow additional time for claims requiring investigation. Incomplete documentation is the most common cause of delays—submitting all required documents with the initial claim helps avoid additional processing time.

F&G requires several documents to process a claim, including Completed Claimant Statement (each beneficiary must submit their own), Certified death certificate issued by the state in which the deceased passed away, and Policy or contract number, and additional documentation depending on the account type. Certified copies are typically needed—photocopies are generally not accepted for death certificates or court documents.


Frequently asked questions

An ILIT is an irrevocable trust that owns a life insurance policy. Because the trust, not the insured, owns the policy, the death benefit proceeds are generally excluded from the insured's taxable estate. To set up an ILIT with an F&G policy, work with an estate planning attorney to create the trust, then have the ILIT trustee complete the Transfer of Ownership form (ADMIN 0177) to assign policy ownership. Trust changes must be accompanied by pages 1 and 2 of the trust and the signature page, and must be signed by the trustees. A 3-year lookback period applies per IRC Section 2035 for transfers of existing policies.

F&G offers living benefits riders on certain life insurance policies that can provide financial assistance if the insured is diagnosed with a qualifying chronic, terminal, or critical illness. To file a living benefits claim, call 866-702-2194, option 5. Living benefits allow access to a portion of the death benefit while the insured is still alive, subject to policy terms and conditions.
