Modern Woodmen lists these among its required documents: Certified (original) death certificate with colored or raised seal; Completed claim form (provided by Modern Woodmen after claim is reported); Government-issued ID for the beneficiary; Certificate number and deceased's information. Most claims process within 5-7 business days after Modern Woodmen receives all required documents. Beneficiaries usually receive payment shortly thereafter. Some claims may take longer depending on documentation completeness and claim complexity.

It depends on the capacity you are acting in. An executor or administrator encloses Letters Testamentary (when there is a will) or Letters of Administration (when there is not); a successor trustee encloses a certificate of trust; a successor under a small estate encloses that state’s small estate affidavit.
