American National lists these among its required documents: Completed Life Claim Form (life insurance) or Annuity Claim Form (annuities); Certified death certificate (original or certified copy); Government-issued identification for the beneficiary; Policy or contract number and deceased's name, date of birth, date of death, and Social Security number.
