Work out which claim track the policy is on - Pacific Guardian Life runs two, and they have different departments, forms, and addresses:
  • INDIVIDUAL life or annuity (Whole Life+, Accumulator 8/15/15 Junior, Lifeguard Elite, Diamond Head MYGA): the beneficiary calls the Claims Department directly
  • GROUP life through an employer: the EMPLOYER or group trust administrator starts the claim on the Group Life Claim Form and it goes to the Group Claims Department, not to Client Relations

For an individual policy, call the Claims Department:

  • Have ready: the insured's name, date of birth, date of death, and policy number(s)
  • If you are not the beneficiary, expect to be asked for the beneficiary's contact information

The claims representative sends a claims packet with a cover letter listing the requirements for that specific policy - Pacific Guardian Life does not publish a downloadable individual claim form, so the packet is the form

Return the packet with the supporting documents:
  • A certified (original) copy of the insured's death certificate
  • The completed claim form from the packet
  • A completed IRS Form W-9 from EACH beneficiary - the claim is not complete until every beneficiary has returned theirs
  • If the beneficiary is the estate: Letters of Administration (or Letters Testamentary) showing the named executor
  • If the beneficiary is a trust: the trust documents and trustee identification
  • If the beneficiary is a minor: documentation designating the legal guardian of the minor's property
  • If the insured resided in a foreign country at the time of death: Pacific Guardian Life states additional requirements will be requested

If a funeral home is to be paid directly from the proceeds, the funeral home completes an Assignment of Life Insurance Proceeds Form and each beneficiary giving up all or part of their benefit signs it; it is submitted with the claim forms

Once all requirements are received FROM EACH beneficiary, Pacific Guardian Life states death claims are processed as quickly as possible, usually within three working days

Group life claims go to the Group Claims Department at 1440 Kapiolani Blvd., Ste 1700, Honolulu, HI 96814 on the published Group Life Claim Form, started by the employer. Beneficiary rules that change the outcome: if the primary beneficiary predeceased the insured, benefits go to the contingent beneficiary, and if there is none, proceeds may be payable to the estate of the deceased insured or to the estate of the deceased primary beneficiary depending on who died first - which can pull an otherwise probate-free death benefit into probate. Minor beneficiaries are never paid directly: benefits go to the legal guardian of the minor's property, and if no guardian has been appointed, Pacific Guardian Life holds the minor's share in an interest-bearing contract until the minor reaches the age of majority. Premium and loan reductions: a death during the 31-day grace period is paid net of the overdue premium, and any outstanding policy loan plus accrued interest reduces the death benefit.

Frequently asked questions

Nothing is paid to the child. Pacific Guardian Life pays benefits to the legal guardian of the minor's property, and asks for the court documentation designating that guardian with the claim. If no guardian has been appointed, the company holds the minor's share of the proceeds in an interest-bearing contract until the minor reaches the age of majority - so the money sits there, and someone has to petition a court to get at it. The Beneficiary Change Request Form gives you the way out: Section 4 lets you name a UTMA custodian and a successor custodian now, and the form warns that if no custodian is named by the time a claim is made, a petition to the court will be required. The form notes UTMA is not available in Vermont or South Carolina. Naming a trust for the child's benefit is the other option.

No - group life runs on a completely separate track. Individual life and annuity claims go to the Claims Department at (800) 432-3306. Group life claims start with the EMPLOYER: the employer or group trust administrator completes the top of the Group Life Claim Form (https://www.pacificguardian.com/wp-content/uploads/2023/07/gl_claim_form.pdf) and the packet goes to Pacific Guardian Life, Group Claims Department, 1440 Kapiolani Blvd., Ste 1700, Honolulu, HI 96814. The form calls for the Certificate of Coverage, an original death certificate, and the ORIGINAL Enrollment Card plus all Change of Beneficiary forms on file - so ask the employer for the complete benefits file, not just the certificate. It also has separate lines for base life, supplemental, accidental death, and dependent life amounts, meaning one employee's file can hold several coverages you are entitled to claim at once.
