There are two ways to keep your MIDFLORIDA accounts out of probate: adding Payable on Death designations and retitling accounts into a trust. Because MIDFLORIDA is a membership-based institution, trust retitling must maintain the membership eligibility requirement. Both methods ensure your accounts transfer directly to the people you choose, without court involvement.

Each of MIDFLORIDA's 15 accounts has different rules for how it transfers at death. The sections below explain the options for each.

Visit any MIDFLORIDA branch with government-issued photo ID

Request a POD beneficiary designation form from a member services representative

Provide beneficiary information:
  • Full legal name, date of birth, Social Security number, and relationship
  • Specify primary and contingent beneficiaries with percentage allocations

Complete and submit the form:
  • Both account owners must sign the form for joint accounts
  • The most recently signed and dated designation form on file controls distribution

Required Documents

  • Trust name, date established, and tax ID (EIN or SSN)

Special Requirements

  • POD designation available for deposit accounts (checking, savings, money market, certificates)
  • IRA beneficiary designations are separate from deposit account POD designations
  • Beneficiary does not need to be present
  • Changes require signatures of all living account owners on the most current form
  • Not available for Student Checking or Youth Savings accounts
