  • Primary beneficiary: full legal name, date of birth, relationship, and percentage allocation
  • Optionally add contingent (secondary) beneficiaries with their details
  • For trusts: enter the full legal trust name, date established, and Trust EIN as the beneficiary

Special Requirements

  • Trusts can be named as primary or contingent beneficiaries
  • Multiple beneficiaries can be named with specific percentage allocations; if percentages are not specified, benefits are divided equally
  • Beneficiary changes can only be made during the lifetime of the insured
  • Completing a new beneficiary form replaces all existing beneficiary designations
  • Irrevocable beneficiary designations require the beneficiary's written consent to change
  • Community property states may require spousal consent for beneficiary changes
  • Use full legal names; avoid generic designations like "my spouse" or "my children" to prevent disputes
  • A trustee of a trust can be named as a beneficiary to allow structured distribution of proceeds
