When a FourLeaf FCU member passes away, the Operations - Decedent Claims handles the transition of accounts to beneficiaries or the estate. Accounts with Payable on Death designations or trust ownership transfer outside of probate, while solely-owned accounts may require Letters Testamentary or Letters of Administration from the probate court.
FourLeaf FCU offers an online claims portal that makes the initial filing process more straightforward. Survivors can also initiate claims by phone or by mailing documentation directly.
Follow these steps to file a death claim with FourLeaf FCU:
Call 844-306-0529 or email EstateServices@fourleaffcu.com to inform FourLeaf of the death before visiting a branch. FourLeaf may also receive notice of a member's death from the U.S. Government and will begin its process upon receipt. A decedent pamphlet with detailed instructions is available at docs.fourleaffcu.com/documents/decendent-pamphlet.pdf. Letters Testamentary/Administration from Surrogate's Court must be dated within 6 months to be accepted. The Deceased Affidavit form is available at fourleaffcu.com/forms/ for small estate claims. Any Power of Attorney held on the account is voided upon the death of the principal. Single-owner accounts with designated beneficiaries are closed and distributed to those beneficiaries; single-owner accounts with no beneficiaries are distributed to the estate; jointly owned accounts remain open for the surviving owner(s) and may be retitled. IRA balances are moved into an Inherited IRA under the beneficiary's name and tax ID.

Documentation required by FourLeaf FCU includes Certified death certificate with raised seal or certified copy (photocopies not accepted), Government-issued photo ID for the claimant, and Letters Testamentary or Letters of Administration bearing seal from Surrogate's Court, dated within 6 months (if no POD designation), along with additional paperwork that varies by account type. All death certificates and court documents must be certified copies.
A Revocable Trust Account, also known as Payable on Death (POD) or Totten Trust, allows you to designate beneficiaries who receive the account funds upon the death of all account owners. If two or more account holders create the account, they own it jointly with survivorship. Beneficiaries cannot withdraw funds while any account creator is living.

For POD/beneficiary accounts, funds are distributed directly to designated beneficiaries.
