When an insured person dies, the beneficiary or executor should contact Farmers Insurance's Life Claims Department at (206) 236-6616 to start the claims process. Insurance proceeds are paid directly to the named beneficiary and do not go through probate. How quickly the claim is processed depends on the policy type, documentation, and cause of death.

To start a claim, contact Farmers Insurance by phone at (206) 236-6616 or email documentation to life.claims@farmersinsurance.com. You will need the account holder's full name, account numbers, and a certified death certificate.

Death claim process

To file a claim after an account holder's death, here is what Farmers Insurance requires:

Filing a claim

1
Notify the Farmers Life Claims Department by calling (206) 236-6616 (collect calls accepted) or the FNWL claims line at 1-800-238-9671, emailing life.claims@farmersinsurance.com, or starting online at https://www.farmers.com/claims/life-claims-form/
2
Provide the policyholder's name, policy number, date of death, and cause of death
3
Farmers will send a claims packet to the contact address, or you may contact your Farmers agent for assistance with the claims process
4
Complete and assemble the required claim documentation:
  • Each named beneficiary must complete and sign a separate Claimant's Statement for Insurance Proceeds (Farmers form MIM 6532); the Life Claims Department verifies the beneficiary designation
  • Obtain a certified death certificate (with raised registrar's seal or multicolored signature seal; photocopies are not accepted)
  • Include the beneficiary's full Social Security number or Taxpayer Identification Number on the claim form (required for processing)
5
If the trust is the beneficiary:
  • The trustee completes the claimant statement
  • Provide a Trustee Certification form and a copy of the trust agreement confirming the trust is still in full force and effect
6
If the beneficiary is an estate:
  • The court-appointed executor completes the claimant statement
  • Provide Letters of Administration or Letters Testamentary
7
If signing as a Power of Attorney, Guardian, or Executor, indicate your title alongside your signature
8
Mail the completed claim forms, certified death certificate, and supporting documents to: Farmers New World Life Insurance Company, P.O. Box 248831, Oklahoma City, OK 73124-8831, or fax to (866) 659-3320
9
Claims processing can take a few weeks to a few months depending on claim complexity and documentation completeness

Required Documents

  • Completed Claimant's Statement for Insurance Proceeds (Farmers form MIM 6532), one per beneficiary
  • Certified death certificate (with raised seal or multicolored signature seal from the issuing authority)
  • Policy number (if available)
  • Beneficiary Social Security number or Taxpayer Identification Number
  • Beneficiary contact information and relationship to the deceased

  • Letters of Administration or Letters Testamentary (if estate claim)
  • Police report, coroner's report, and/or toxicology report (if death was due to accident or homicide)
  • Medical authorization / HIPAA form (if requested for contestable claims within the first two policy years)

What to know at this institution

The Life Claims Department can be reached directly at (206) 236-6616 (collect calls accepted), the FNWL claims line at 1-800-238-9671, by email at life.claims@farmersinsurance.com, or by fax at (866) 659-3320. A claim can also be started online at https://www.farmers.com/claims/life-claims-form/. You can also call the general Farmers claims line at 1-800-435-7764 or contact your Farmers agent for assistance. For graded benefit whole life policies, non-accidental death within the first two years results in a benefit limited to 110% of premiums paid rather than the full death benefit. Contestable claims (death within the first two policy years) may require additional medical records and authorization. Claims are processed upon receipt of all required documentation. Farmers agents can serve as a local point of contact throughout the claims process.

Prepare your letter of instruction to Farmers Insurance

Farmers Insurance asks for a letter of instruction alongside its claim form. We prepare a transmittal cover letter and the enclosure checklist Farmers Insurance requires.

Build your letter of instruction

Expected timelines at Farmers Insurance: A few weeks for straightforward claims; may take several months for complex or contestable claims. Delays are almost always caused by incomplete paperwork—gathering all required documents before filing the initial claim helps avoid back-and-forth.

Documentation required by Farmers Insurance includes Completed Claimant's Statement for Insurance Proceeds (Farmers form MIM 6532), one per beneficiary, Certified death certificate (with raised seal or multicolored signature seal from the issuing authority), and Policy number (if available), along with additional paperwork that varies by account type. All death certificates and court documents must be certified copies.

Frequently asked questions

An ILIT is created when you transfer ownership of a life insurance policy to an irrevocable trust. The trust owns the policy, and the trustee manages it while the insured is alive. When the insured dies, the death benefit is paid to the trust and distributed according to its terms. Because the trust owns the policy, the death benefit is generally excluded from the insured's taxable estate. The grantor loses ownership and control once transferred. Complete the Farmers Transfer of Ownership / Assignment form and provide a copy of the trust agreement. Permanent life insurance (whole life, universal life) is preferred for ILITs because term policies may expire before the insured's death.

Notify the Farmers Life Claims Department at (206) 236-6616 (collect calls accepted), the FNWL claims line at 1-800-238-9671, by email at life.claims@farmersinsurance.com, or by starting online at https://www.farmers.com/claims/life-claims-form/. You can also contact your Farmers agent for assistance. Provide the policyholder's name, policy number, date of death, and cause of death. Farmers will send a claims packet to the contact address. Each beneficiary must complete and sign a separate Claimant's Statement for Insurance Proceeds (form MIM 6532) and obtain a certified death certificate with a raised seal. Mail completed documents to: Farmers New World Life Insurance Company, P.O. Box 248831, Oklahoma City, OK 73124-8831, or fax to (866) 659-3320.

Farmers New World Life Insurance Company offers five types of life insurance: (1) Simple Term life insurance with 10, 20, or 30-year terms and no medical exam for $75,000-$150,000 coverage; (2) Value Term life insurance with 10, 15, 20, or 30-year terms starting at $150,000; (3) EssentialLife Simple whole life with guaranteed cash value starting at $15,000; (4) Graded Benefit whole life for $5,000-$30,000 with guaranteed acceptance; (5) EssentialLife Universal life with flexible premiums starting at $50,000; (6) Index Universal Life with S&P 500 and Russell 2000 indexed accounts; and (7) Accidental Death insurance from $37,500 to $200,000.

Farmers New World Life Insurance Company sells life insurance in all states except New York. Product availability and provisions may vary by state. Accidental death insurance is currently available for purchase online in 21 states (AL, AR, AZ, CA, GA, IA, IN, KS, MO, NC, ND, NE, NM, NV, OR, SD, TN, TX, UT, WI, and WY). All Farmers life insurance policies are sold through Farmers Insurance agents; there is no direct online purchase option for other life insurance products.

Farmers Accidental Death Insurance provides a death benefit only if the insured dies as a result of a covered accident. No benefits are paid for death from sickness, disease, or natural causes. Coverage amounts range from $37,500 to $200,000, with proceeds up to $1,000,000 in certain circumstances such as death as a fare-paying passenger on a commercial airplane. No medical underwriting is required. The policy is guaranteed renewable and non-cancellable until age 80 (age 75 in California), with coverage reduced after age 65 per the terms of the contract. It is currently available for purchase online in 21 states.

Yes. A revocable living trust can be named as a primary or contingent beneficiary on any Farmers New World Life Insurance policy. Complete the Change of Beneficiary form with the trust name, date established, trustee name, and trust Tax Identification Number (EIN). Naming the trust as beneficiary directs the death benefit into the trust at the insured's death for distribution per the trust's terms, without transferring policy ownership. This approach does not remove the policy from the insured's taxable estate; for that purpose, an Irrevocable Life Insurance Trust (ILIT) is required. Mail completed forms to Farmers New World Life Insurance Company, P.O. Box 248831, Oklahoma City, OK 73124-8831.

All Farmers New World Life Insurance forms, claim documents, and trust paperwork should be mailed to: Farmers New World Life Insurance Company, P.O. Box 248831, Oklahoma City, OK 73124-8831. Documents can also be faxed to (866) 659-3320. For assistance, call Farmers New World Life customer service at 1-800-238-9671 (Monday-Friday business hours) or contact the Life Claims Department at (206) 236-6616 or life.claims@farmersinsurance.com. A life claim can also be started online at https://www.farmers.com/claims/life-claims-form/. The general Farmers claims line at 1-800-435-7764 is available 24/7. Your local Farmers agent can also serve as a point of contact for submitting paperwork.

Farmers Insurance's Life Claims Department can be reached by phone at 1-800-238-9671, email at life.claims@farmersinsurance.com, and fax at (866) 659-3320 for questions throughout the claims process.

When the deceased had multiple Farmers Insurance policies, some may need separate claims while others can be handled together. The Life Claims Department can clarify what's needed for each account type.
