When a Affinity Plus member passes away, the Deceased Member Care Team handles the transition of accounts to beneficiaries or the estate. Accounts with Payable on Death designations or trust ownership transfer outside of probate, while solely-owned accounts may require Letters Testamentary or Letters of Administration from the probate court.

Affinity Plus offers an online claims portal that makes the initial filing process more straightforward. Survivors can also initiate claims by phone or by mailing documentation directly.

Death claim process

The death claim process at Affinity Plus works as follows:

Filing a claim

1
Review the deceased member guidance at https://www.affinityplus.org/deceased-member-guidance
2
Notify Affinity Plus of the member's death through any of the following channels:
  • Call (800) 322-7228 or (651) 291-3700
  • Visit any branch location
  • Chat or video call during business hours
  • Mail documentation to the Deceased Member Care Team
3
Provide initial information:
  • Deceased member's name
  • Your name and relationship to the deceased
  • Your contact information (phone/email) and preferred contact method
4
Submit a certified copy of the death certificate (must include seal or statement reading "This is a true and certified copy" or watermark)
5
Settlement depends on the account structure:
  • Joint accounts: surviving owner retains the account with right of survivorship
  • POD accounts: named beneficiaries receive funds upon providing required documentation
  • Trust accounts: successor trustee provides trust documents, death certificate, and Tax ID Number
  • Accounts without beneficiaries or joint owners: personal representative provides Letters Testamentary (if will exists) or Letters of Administration (if no will) from probate court

Required Documents

  • Certified death certificate (with official seal, certification statement, or watermark)
  • Government-issued photo ID of the claimant(s)
  • Letters Testamentary (if will exists and probate is required)
  • Letters of Administration (if no will exists and probate is required)
  • Trust documents and Tax ID Number (if trust-owned account)
  • Small Estate Affidavit (if estate qualifies under Minnesota law; threshold is $75,000)

What to know at this institution

Minnesota estates with assets valued at $75,000 or less may qualify for an Affidavit of Collection of Personal Property (small estate affidavit), allowing inheritors to claim property without going to probate court. For estates exceeding $75,000, probate proceedings may be required. The Deceased Member Care Team handles all death claim matters and can be reached at (800) 322-7228 or by mail at 175 West Lafayette Frontage Road, St. Paul, MN 55107.

Prepare your letter of instruction to Affinity Plus

Affinity Plus accepts a claimant-drafted letter of instruction. We draft it for you — addressed to Affinity Plus's verified claims department, with the documents it requires enclosed.

Build your letter of instruction

Processing timelines at Affinity Plus: Joint accounts transfer immediately upon notification. POD beneficiaries typically receive funds after submitting required documentation. Accounts requiring probate depend on Minnesota probate court timelines. Mail-based processing takes longer than direct contact. Affinity Plus provides a personalized timeline based on each situation. Incomplete documentation is the most common cause of delays—submitting all required documents with the initial claim helps avoid additional processing time.

Affinity Plus requires several documents to process a claim, including Certified death certificate (with official seal, certification statement, or watermark), Government-issued photo ID of the claimant(s), and Letters Testamentary (if will exists and probate is required), and additional documentation depending on the account type. Certified copies are typically needed—photocopies are generally not accepted for death certificates or court documents.

Frequently asked questions

Contact the Deceased Member Care Team at (800) 322-7228, visit any branch, or mail documentation to 175 West Lafayette Frontage Road, St. Paul, MN 55107. You will need a certified death certificate with an official seal, certification statement, or watermark. Joint accounts transfer immediately to the surviving owner with right of survivorship. POD (Payable on Death) accounts pay to named beneficiaries upon providing required documentation. Trust accounts transfer to the successor trustee with trust documents and a death certificate. Accounts without beneficiaries or joint owners require Letters Testamentary or Letters of Administration from probate court. Minnesota estates valued at $75,000 or less may qualify for a small estate affidavit.

Download the Beneficiary Request Form from affinityplus.org or request one from a branch. Complete it with each beneficiary's full name, address, date of birth, Social Security number, and relationship to the accountholder. The form covers up to four beneficiaries and has no percentage-allocation field; instead you mark each beneficiary as applying to all shares on the membership (now and in the future) or only to specific Share ID(s). Submit via secure message in digital banking, mail to 175 West Lafayette Frontage Rd, St. Paul, MN 55107, fax to (651) 312-6252, or drop off at any branch. IRA and HSA accounts require separate beneficiary designation forms. Beneficiary designations on deposit accounts create a POD (Payable on Death) arrangement that bypasses probate.

Affinity Plus's Deceased Member Care Team can be reached by phone at 1-800-322-7228 and fax at 651-312-6252 for questions throughout the claims process.

If the deceased held multiple Affinity Plus accounts, each may require a separate claim or have different documentation requirements. The Deceased Member Care Team can confirm which accounts require individual attention and which can be processed together.
