How to request a transfer

To request a transfer of Verizon accounts after an account holder's death, follow these steps:

Decide whether to keep the deceased's phone number on a Verizon account or close the account. Gather what Verizon asks for before you call or submit the form:
  • The Account Owner's name
  • The mobile phone number on the account
  • The Verizon mobile Account PIN (if you have it)
  • A death certificate and your executor paperwork (Letters Testamentary or Letters of Administration)

To KEEP the number, transfer the account to a new or different Verizon account. Complete Verizon's "Executor transfer or disconnect service form" (a surviving account member can instead use the account-member transfer path). See What to do when someone on your mobile account passes away.
  • Verizon reviews the request in about 3 business days
  • After review, the new Account Owner is emailed to complete the transfer

To CLOSE the account or disconnect a line, call Verizon at 800-922-0204 or visit a Verizon company store with the death certificate and executor paperwork. Requests are reviewed in about 3 business days.

Resolve any device payment-plan or device-lease balance on the account. Verizon offers two options:
  • Return the device: Verizon sends a return kit, and when the device is received the remaining balance is forgiven
  • Keep the device: the line is disconnected and the remaining device payment balance is charged on the next bill

Understand that rewards are forfeited. Verizon Dollars and Verizon Up / myAccess benefits are non-transferable, and terminating the service forfeits any accumulated Verizon Dollars. They cannot be cashed out or moved to another person before closing the account.

If you want to keep the phone number but not on Verizon, port it to another carrier from the new account after the transfer, rather than letting the line disconnect.
