  • Sign in or register for your online life insurance account at johnhancock.com
  • Select your policy from the dashboard
  • Navigate to the "Policy" tab and select the "Policy overview" subtab

  • View current beneficiaries and request changes
  • Add or update primary and contingent beneficiaries with percentage allocations totaling 100%
  • For trusts: provide the trust name, date established, and EIN

Special Requirements

  • Trusts can be named as primary or contingent beneficiaries
  • Multiple beneficiaries can be named with percentage allocations
  • Per stirpes designations available for family beneficiaries
  • All contract owners and trustees must sign the form
  • If a trust or trustee has changed, updated trust documentation must be provided
  • Minor children cannot receive payouts directly; a trust or custodian must be designated
  • If you do not receive acknowledgment of annuity beneficiary changes within 15 calendar days, contact John Hancock to confirm receipt
