Contact your local Erie Insurance agent or call Erie Family Life at 800-458-0811, option 3 (Monday-Friday, 8:00 AM - 5:30 PM ET) to request a beneficiary change form. Complete the form with each beneficiary's full legal name, date of birth, relationship, and percentage allocation. Percentage allocations must total 100% for each tier. Submit the completed form through your agent or mail to Erie Family Life Service Center, P.O. Box 83026, Lincoln, NE 68501-3026, or fax to 866-567-1219.
Erie Family Life offers three categories of life insurance. Traditional term life insurance is available in 5-, 10-, 15-, 20-, and 30-year terms with a $30,000 minimum coverage and medical exam required. Whole life insurance includes the Whole Life Protector (for protection) and Whole Life Accumulator (for cash value growth) with coverage from $10,000 to $10,000,000 and six payment duration options. ERIExpress is a no-exam product line with coverage from $10,000 to $500,000, available in both term (5-, 10-, 15-, 20-year) and whole life (20-year pay) options. All products can have a trust named as beneficiary.
Yes. Erie term life insurance policies include a conversion privilege that allows you to convert to permanent life coverage during the level period or before age 70, whichever comes first. The conversion is guaranteed regardless of any changes in your health or occupation. The replacement permanent policy face amount cannot exceed the original term policy amount. Contact your Erie Insurance agent to discuss conversion options.
