Request to add or update a Payable on Death (POD) beneficiary designation:
- Provide beneficiary full legal name, date of birth, and Social Security number
- Specify relationship to account holder
- Designate primary and contingent beneficiaries with percentage allocations totaling 100%

Sign the completed POD beneficiary designation form.

Keep a copy of the signed form for your records.
