View details →The T. Rowe Price Trustee Certification form states on its face that it may not be used for trusts created in five states: Connecticut, Louisiana, New York, Oklahoma, and Rhode Island. Those states impose their own certification-of-trust statutes, so T. Rowe Price will not take its standard form for a trust formed there. Everywhere else, the Trustee Certification form is the ONLY document T. Rowe Price accepts to certify a trust when you are adding or changing trustees on an existing trust account -- it says it will not accept any other document for that purpose. If your trust was created in one of the five excluded states, call 1-800-225-5132 before mailing anything and ask which certification T. Rowe Price will take.
Retitling always takes two forms, and the same pair covers both mutual fund and brokerage accounts: the Ownership Change form to move the assets out of your individual registration, and a New Account form (Mutual Fund or Brokerage) opening the account in the name of the trust -- the Ownership Change form states that changing to a trust or changing the primary taxpayer identification number requires a New Account form. Do not use the Brokerage Transfer form; it is only for moving assets in from another firm and says on its face that it cannot be used to change ownership. You also submit trust pages showing the trust name, date, current trustees, and their signatures. These must be originals, certified copies, or bear a raised seal -- photocopies are rejected. A Medallion signature guarantee is required, and T. Rowe Price will not accept one from a notary public. If the brokerage trust account will use margin or options, add the notarized Brokerage Trustee Powers form, which mails to a different box (P.O. Box 17435, Baltimore, MD 21297-1435).
