You can also download the Beneficiary Change Request form (Form F58397) from securian.com and mail it to Securian Financial, PO Box 64114, St. Paul, MN 55164-0114. For assistance, call the Transaction Line at 1-800-643-5728. You can designate primary and contingent beneficiaries with percentage allocations totaling 100%. Provide each beneficiary's full legal name, date of birth, Social Security number, and relationship. Completing the form revokes all prior beneficiary designations.
