Only if you live in a community property state. On the POL-08 Beneficiary Add / Change Form used for Primerica Shareholder Services accounts, the spousal consent block (Section 3) applies solely to residents of AZ, CA, ID, LA, NV, NM, PR, TX, WA, and WI. If you live in one of those and your spouse is NOT the sole primary beneficiary, your spouse must sign in front of a notary. Everywhere else the section is skipped entirely, and it never applies to a Coverdell ESA or an inherited/Beneficiary account. The practical trap: a POL-08 that requires a notary CANNOT be faxed to Primerica Shareholder Services. The (833) 782-4119 fax line is only for forms without a notary; a notarized form has to be mailed to P.O. Box 534485, Pittsburgh, PA 15253-4485.
