Complete the form with each beneficiary's full legal name, date of birth, Social Security number, relationship, and percentage allocation. The policy owner must sign and date the form. If there is an irrevocable beneficiary, their signature is also required. Policy change requests are generally processed within 15 business days after receipt.

National Benefit Life Insurance Company is a wholly owned subsidiary of Primerica Life Insurance Company. Both companies are part of the Primerica, Inc. The key difference is geographic: National Benefit Life underwrites Primerica term life insurance policies issued in New York State, while Primerica Life Insurance Company underwrites policies in all other states. National Benefit Life offers term life insurance with level premium periods of 10, 15, 20, 25, 30, and 35 years through the Custom Advantage product line. A simplified-issue option called TermNow is also available and does not require a medical exam. For 10, 15, 20, and 25-year policies, premiums are guaranteed level for the entire term. For 30 and 35-year policies, premiums are guaranteed level for the first 20 years and may increase thereafter. Availability of specific term lengths depends on the insured's age at issue.

National Benefit Life generally processes policy change requests within 15 business days after the request is received. This includes beneficiary changes, ownership transfers, address changes, and name changes. Processing times may vary if additional documentation is required or if the request is incomplete.
