Request a beneficiary change:
  • Provide your policy or contract number and personal identification
  • A representative will assist you and can mail the Beneficiary Designation form (OL122A)

Required Documents

  • Trust name, date established, and tax ID (EIN or SSN)

Special Requirements

  • Name the trust itself as beneficiary, not the individual trustee
  • If a trust is designated as beneficiary, complete the Certification and Acknowledgement of Trust Agreement form (OL4132A)
  • Percentage allocations must total 100% for each beneficiary tier
  • Beneficiary designations on insurance policies and annuity contracts override instructions in a will
  • Only the policy or contract owner can change beneficiary designations
  • Spousal consent may be required in community property states
