  • If an irrevocable beneficiary was previously named, their written consent is required for any change
  • The form must be signed by the person who has the right to change the beneficiary under the policy terms
  • Beneficiary designations override instructions in a will
  • Changing a beneficiary does not change the ownership of the policy
