Mutual of Omaha offers 15 insurance products that play a central role in estate planning. Because policy proceeds pass directly to named beneficiaries—bypassing probate entirely—keeping designations current is one of the highest-impact steps policyholders can take.

Mutual of Omaha lets account holders update beneficiary designations online, by mail, and by phone, typically taking 10-15 minutes online; 15-20 minutes by mail. Trust funding is also available, allowing families to name a trust as the policy beneficiary or establish an irrevocable life insurance trust (ILIT).

Log in to your Mutual of Omaha account online to manage beneficiaries, or download the Application for Change of Beneficiary (Form L4237) from mutualofomaha.com/support/forms. Complete the form with your primary and contingent beneficiaries, including full legal names, addresses, dates of birth, relationships, and percentage allocations totaling 100%. Submit the form by mail to Mutual of Omaha, Policyowner Services, 3300 Mutual of Omaha Plaza, Omaha, NE 68175, or by fax to 402-997-1906. For annuity beneficiary changes, use Form L8529 instead. You can also call 800-775-6000 or 800-775-1000 for assistance.

Yes. You can transfer ownership of a Mutual of Omaha life insurance policy to an ILIT using the Change of Ownership form (Form L6501). The trustee of the ILIT should be the applicant and owner of the policy, and the insured cannot serve as trustee of an ILIT that owns a policy on their life. If an existing policy is transferred to an ILIT, the three-year lookback rule under IRC Section 2035 applies for estate tax purposes. You must include pages from the trust document showing it has been executed and identifying the trustee(s) and successor trustee. Mutual of Omaha does not establish ILITs; consult an estate attorney to create one.

Yes, for non-qualified annuities (Ultra Premier, Ultra Advantage). Use Form L8529 (Annuity Change of Ownership) to transfer ownership to a trust. If the new owner is a trust, provide the trust name, trustee name, and include pages from the trust showing it has been executed. The change of ownership of an annuity may have tax consequences, so consult your tax advisor before making this change. The completed form can be mailed to Mutual of Omaha, Attn: Policyowner Services, Individual Annuity Services, Omaha, NE 68175 or faxed to 402-997-1906. Immediate annuities (Ultra Income) cannot be re-owned once annuitized.

Mutual of Omaha sold Mutual of Omaha Bank to CIT Group in January 2020, and CIT was then acquired by First Citizens BancShares in January 2022. Mutual of Omaha Insurance Company no longer offers retail banking products. Deposit-account inquiries (checking, savings, CDs, mortgages originated under the Mutual of Omaha Bank brand) should be directed to First Citizens Bank. Mutual of Omaha still offers life insurance, annuities, Medicare supplement, long-term care, and critical illness coverage through Mutual of Omaha Insurance Company and its subsidiary United of Omaha Life Insurance Company.
