Mid Atlantic Trust Company (MATC), now doing business as American Trust Custody, is a South Dakota chartered non-depository trust company that acts as trustee or custodian on retirement plans — it is not a bank, it has no branches, and it is not the company that administered your relative's plan. In its own words, "MATC is the custodian and does not have any participant level information." So it cannot tell you a balance, who the beneficiary is, or why a distribution happened. What it CAN do is tell you who the plan's third-party administrator or recordkeeper is, and reissue a check. Call MATC Cashiering at (800) 693-7800 or email requests@macg.com with the exact dollar amount, the check number if you have one, and the date on the check. A 1099-R means a distribution or rollover was processed in the prior tax year; a Form 5498 means an IRA was open for that person's benefit. Either one is a thread worth pulling if the estate cannot find the account. The TPA's phone number is usually printed at the bottom of the tax form itself.
