Insurance proceeds bypass probate entirely and go directly to whoever is listed—regardless of what a will says.   • Full legal names, dates of birth, relationships, and percentage allocations totaling 100%
  • For trusts: provide the full trust name, date established, and trust tax ID number (EIN)

Special Requirements

  • Trusts can be named as beneficiaries on life insurance and annuity products
  • Primary and contingent beneficiaries allowed with per stirpes designations available
  • Percentage allocations must total 100% for each beneficiary tier
  • Spousal consent may be required for certain products depending on state law
  • Complete a separate form for each policy unless owner and all info is identical
  • Beneficiary designations override instructions in a will
  • For annuities, review your contract to determine if the death benefit is payable on death of the owner or the annuitant
