Log in to your Lincoln Financial account online, navigate to your policy details, select "Beneficiary Information" from the top tab, and click "Manage Beneficiaries" to view and update designations. You can also download the Beneficiary Change form (Form CS06893 for life insurance, Form 28493 for annuities) from lincolnfinancial.com and submit by mail to Lincoln Financial Group, PO Box 21008, Greensboro, NC 27420-1008, by fax to 800-819-1987, or by emailing a scanned copy to CustServSupportTeam@LFG.com. You can also call 800-487-1485 for assistance. Complete a separate form for each policy unless the owner and all information is the same.

Yes. Lincoln Financial accepts trust ownership of annuity contracts. The trustee must complete the Certification of Trustee Powers for Annuities form (AN07086) -- note this is a different form than the life insurance version (CL11702). The form certifies the existence of the trust and the identity and powers of the trustees. Lincoln reserves the right to require the full trust document and any subsequent amendments or restatements. A new AN07086 must be submitted whenever the Trustee(s), Grantor information, or Trust TIN changes. Lincoln does not accept a Power of Attorney acting on behalf of a Trustee -- only the Trustee(s) and Successor Trustee(s) may act and sign on behalf of the Trust. When a trust is named as annuity beneficiary, the available payout options are limited to lump sum, 5-year deferral, or continuation of i4LIFE on a nonqualified contract. Tax consequences of trust-owned annuities depend on whether the trust is a grantor or non-grantor trust; consult a tax advisor.
