Estate planning with Integrity Life policies centers on beneficiary designations—the single most important step for ensuring life insurance proceeds and annuity benefits reach the intended recipients without probate involvement. Unlike bank or brokerage accounts, insurance products are not retitled into trusts; instead, trusts are named as beneficiaries when estate tax planning or controlled distributions are needed.

Integrity Life has 13 product types, and the estate transfer rules differ across them. Some support beneficiary designations, some can be retitled into a trust, and others will require probate if nothing is set up. Each is covered below.

Provide your contract number and verify your identity

Ask specifically whether you are changing the ANNUITANT's beneficiary, the OWNER's beneficiary, or both — Integrity treats them as separate designations on separate sections of the form

Request the Annuitant Beneficiary Designation Form (IL-61-0002) and/or the Non-Financial Service Request (IL-00-3185) be sent to you, or download them from https://www.westernsouthern.com/distributors/resources

Complete, sign, and return the form by mail or fax — Integrity does not accept a beneficiary change over the phone

Required Documents

  • Trust name, date established, and tax ID (EIN or SSN)

Special Requirements

  • No online beneficiary changes — Integrity requires a signed paper form returned by mail or fax (888-220-2677)
  • Two distinct designations: the annuitant's beneficiary (IL-61-0002) and the owner's beneficiary (IL-00-3185, Section 4). Where the owner and annuitant are different people, updating one does NOT update the other
  • All owners must sign; unless marked contingent, every named beneficiary is treated as a primary beneficiary
  • Spousal consent required where the owner resides or has resided in AZ, CA, ID, LA, NM, NV, TX, WA, or WI, unless the spouse is the annuitant, owner, or sole primary beneficiary
  • Spousal consent also required on an IRC 401(a) contract subject to ERISA when a non-spouse is named
  • If the contract is owned by a trust or other entity, that same trust or entity must remain the contract beneficiary — you cannot leave an individual as beneficiary of a trust-owned contract
  • The owner may restrict the beneficiary's payout (full or partial restriction to an annuity option), but a spousal beneficiary may still have the right to re-register the contract regardless of the restriction elected
