Complete the Beneficiary Designation Form and return it by mail or in person to any Golden 1 branch along with a copy of valid government-issued photo ID for all account owners. All account owners must sign the form. Completing the form cancels all previous beneficiary designations for the account. Beneficiary changes cannot be made online or by phone. Download the form at golden1.com or request one at any branch.
Beneficiaries are only recognized if the decedent designated them on the Golden 1 account application or Beneficiary Designation Form. Without a POD designation, Golden 1 can only take instructions from the court-appointed legal representative (executor or administrator). The account will need to go through probate or qualify for a Small Estate Affidavit.
Golden 1 accepts a Certification of Trust under California Probate Code Section 18100.5 or the full trust document with permissible excerpts and amendments. Bring the certification or trust pages, valid government-issued ID for each trustee, the trust EIN (or grantor's SSN for a revocable grantor trust), and complete the Application for Membership and Trust Account Agreement (Form C-3). Trust accounts can be opened in branch or by mail; checking, savings, money market, and certificates can all be titled to the trust. Call 1-877-465-3361 to confirm requirements before visiting a branch.
