CareScout Insurance Company also writes Care Assurance, a standalone LTC insurance product launched in October 2025 that is available in 40 states as of February 2026 and includes access to the CareScout Quality Network.

Download the state-specific Ownership and Beneficiary Designation Request form from genworth.com/customer-support/forms-and-documents/life-insurance-forms. There are two versions: one for policies delivered in CA, CT, DE, DC, FL, NY, ND, and SD, and a separate version for all other states. Complete all required fields including primary and contingent beneficiary information with full legal names, dates of birth, Social Security numbers, relationships, and percentage allocations. Sign, date, and submit the form by mail to the address on the form, by email to LifeCustomerService@genworth.com, or by fax. You can also call 888-GENWORTH (888-436-9678) to request the form.

Yes. You can name a trust as beneficiary on a Genworth annuity using the Annuity Contract Change Form, available at genworth.com/customer-support/forms-and-documents/annuity-forms. Provide the full trust name, date established, trustee name, and EIN. Designations must be made in percentages. Even if only changing the contingent beneficiary, you must restate the primary beneficiary. For trust-owned annuity contracts, documentation showing signing authority is required. Note that naming a non-spouse beneficiary or trust may limit tax-deferral options. For annuity changes, you may also log in to your MyGenworth account at genworth.com/login.
