Request a beneficiary change form for your life insurance or annuity policy

- Trust name, date established, and tax ID (EIN or SSN)
Special Requirements
- Only the policy owner can change the beneficiary designation
- The beneficiary change form requires a witness signature
- If naming a trust as beneficiary, provide the full legal name of the trust, the date the trust was established, and the trustee name
