Log in to your account at portal.equitable.com, navigate to "Manage your account," and click "Forms" to access the beneficiary change form. Alternatively, download the Life Insurance Beneficiary Change form from equitable.com or request it by calling (800) 777-6510. Complete and submit by mail to PO Box 1047, Charlotte, NC 28201-1047 or fax to (855) 268-6378.

Contact Equitable customer service or your financial professional to request the appropriate Beneficiary Designation Form. For EQUI-VEST products, call (800) 628-6673. For SPDA products, call (800) 628-7789. You can also log in to portal.equitable.com to access forms online. Completed forms can be submitted by mail or fax.

Yes. Life insurance policies including variable universal life, indexed universal life, and term life can be assigned to an ILIT. Request the Life Insurance Ownership Change form. You must provide the Trust Certification or relevant trust agreement pages, trustee details, and trust TIN.
